*Job Title: *Physical Therapy Assistant*Facility Type: *Skilled Nursing/Assisted Living Facility*Position Type: *Full-Time, Permanent*Salary: *Starting From $32.00 per hour plus full benefits*Reports To: *Director of Rehab*Overview:*We are seeking a dedicated and skilled Physical Therapy Assistant to join our team at skilled nursing/Assisted Living facility that offers both long-term and short-term rehabilitation services.*Responsibilities** Assist licensed physical therapists in implementing treatment plans for patients in various settings, including outpatient clinics and skilled nursing facilities.* Provide direct patient care by administering therapeutic exercises and modalities.* Monitor patient progress and document treatment outcomes.* Educate patients and their families on care plans and home health strategies.* Collaborate with healthcare teams to ensure comprehensive patient care and effective discharge planning.*Qualifications:*To be considered for this role, candidates must meet the following requirements:* 1.
