Overview: Summary:
Performs specific patient care procedures (such as basic wound care
and catheter maintenance) and tasks under the direction of the
primary registered nurse or other licensed clinician. All
procedures, duties, and tasks are to be completed under the
patients care plan as ordered in conjunction with the physician.
Patient care primary setting is in the patient home which can
include single family residence, Assisted Living Facilities (ALF),
or Nursing Home. Patient level of care can range from skilled Home
Healthcare, Palliative, and/or Hospice. Type of Opportunity: Per

Responsibilities:
Works clinically under the direction of a registered nurse or
licensed clinician while independently providing care in the
patient home setting.
Plans and navigates route to patient service address based on
road/traffic conditions utilizing driving applications when
necessary.
Manages and maintains patient caseload and patient scheduling to
ensure frequency/duration of ordered care is delivered timely while
taking into account patient preference.
Monitors, collects, and records information in the patient record.
Collects and records vital signs, skin, bowel and bladder function,
tolerance to activities, and other patient data, as ordered in the
CNA careplan.
. Reports any significant problems or changes in patient status to
the primary RN/clinician.
Performs personal care and assists with activities of daily living
such as personal hygiene (shaving, mouth care, foot care,
peri-care, and bathing) and emotional support.
Assists with restorative aide duties for feeding patients and
report on tolerance.
Performs basic wound care and catheter maintenance, as ordered and
directed by the RN case manager
Applies oxygen delivery devices as ordered without altering
settings or determining rates. Assists patients with portable
oxygen, oxygen concentrator, humidifier, and ensures oxygen safety
measures.
Assists patient with bed and commode transfers, ambulation with or
without equipment, including Hoyer lift.
Assists patient with prescribed exercises including range of motion
(ROM).
Assists patients with taking medications by providing verbal
cues/prompts to take oral medications which are labeled in a
container, with dose and time, set up by patient, family or
nurse.
Follows written treatment care plan as established by the team
offering suggestions for modification. Actively participates in IDT
and Case Conferences.
Reinforces teaching of patient and family/caregivers, such as
personal care techniques, including care of skin, repositioning and
transferring following training by R.N. or therapist.
Clinical documentation of care according to agency policy,
regulations and standards.
Provides simple meal preparation, cleaning of work area, laundry
and respite care as ordered.
Participates in agency continuous quality improvement activities as
required.

Qualifications: Other information:
High School diploma or equivalent.
1 year of direct patient care in any clinical setting, or in-home
health/hospice is preferred.
CNA (Certified Nursing Assistant) certification
BLS
Microsoft Office
Electronic devices, laptop, desktop, smartphone, program
applications
Electronic Medical Record on various platforms
Ability to learn and document in EPIC charting system
Good driving record, as defined by Presbyterian, access to a
vehicle, drivers license, auto insurance
