Job Description:
Overview: Presbyterian is seeking a Nurse Referral Coordinator
to provide direct patient care to an assigned group of patients in
an Outpatient Clinic. Common responsibilities include facilitating
and following the cardiac patient from point of referral and then
coordinating arrangements for referral care, procedural care and /
or long-term cardiac care. The navigator is the contact person for
the patient /family and serves as the liaison to all physicians and
Healthcare staff providing care and support for the patient. Type

- EPIC electronic charting system Responsibilities:
- Facilitates the entry of patients into the cardiology service
line based on cardiac diagnosis and coordinates care including
family involvement. Follows the patient throughout the continuum of
cardiac care services across multiple departments.
- Expedites and facilitates patient consultation and referral
requests by managing all referral work queues including daily
review of RN referral pool in EMR, enters referral order, performs
chart and medical record review, enters orders in EMR for requested
testing e.g. monitors/stress test/echos, provides scheduling
directions in referral and routes to scheduling
- Accepts warm transfers/ physician calls and serves as the
primary point of contact for referring providers
- Coordinates care for patients using established PHG referral
guidelines
- Acts as a PHG liaison to outside referring facilities to assist
with care coordination including requesting pertinent medical
records.
- Implements risk assessments, healthcare and quality of life
pathways for cardiac patients, based upon the clinical staging of
the patient. Evaluates progress of patients along the continuum and
is responsible for professional and personal modifications along
with healthcare team as appropriate.
- Develops relationships with cardiology providers and other
providers including but not limited to: interventional
cardiologist, electrophysiologist, radiologist, cardiothoracic
surgeon, vascular surgeon, medical oncologist, primary Care
Physicians and serves as a liaison between patient and
physicians
- Provides patient education across the continuum of care and
provides symptom management and test results for patients
- Assists with tracking of metrics and quality indicators for
diagnosis and scheduling patients working with PHG schedulers and
population health
- Delegates or assigns aspects of patient care to healthcare team
members commensurate with their validated competency to perform
task
- Assumes professional responsibility for current nursing
structure and clinical standards of care
- Assumes other leadership roles as required by the
department
- Performs other functions as assigned Qualifications:
- Registered Nurse (RN) - State of New Mexico or Compact State
Nursing License required.
- Associate degree in nursing
- BLS Certification required at time of hire.
