This role ensures compassionate, respectful care in alignment with established care plans and patient needs.*RESPONSIBILITIES** Adhere to agency policies and procedures to ensure the highest standards of patient care.* Provide assigned personal care tasks, including bathing and hands-on support, with compassion and professionalism.* Maintain timely and accurate documentation for each visit, in compliance with agency standards.* Communicate any changes in patient condition to the supervising RN promptly.* Assist with household tasks essential to patient health and well-being, as assigned.* Support patient mobility and physical activity as directed, including ambulation and exercise.* Perform basic therapeutic procedures, such as range-of-motion exercises, under supervision.* Assist with patient medications as directed for self-administered medications.* Participate in required in-service training and continuing education to maintain skills.* Engage in patient care conferences and collaborative discussions as scheduled.Requirements:*QUALIFICATIONS** Completion of a Medicare-approved training program for Home Health Aide and/or Hospice Aide, with demonstrated competency.* Compassionate and respectful attitude towards the care of the elderly and those who are ill.* Proficiency in following directions, as well as reading and writing.* Emotional maturity and resilience to meet the demands of patient care.*WORKING ENVIRONMENT*Primarily works in patient homes with occasional tasks completed at the agency office.
